| • 21-40 Years The upper age limit relaxation is as follows: • 5 years for OBC (Non-Creamy Layer), SC, and ST candidates. • 15 years for candidates with disabilities. • 3 years plus the time served in the Army for Emergency Commissioned Officers, Short Service Commissioned Officers, and Ex-Army Personnel from Uttar Pradesh. • If candidates qualify for age relaxation in multiple categories, they will receive only the most beneficial concession. | |||||
| UP NHM CHO Recruitment 2024: Eligibility Criteria | |||||
| 1. Education: Candidates must have completed a B.Sc. (Nursing) or Post Basic B.Sc. (Nursing) with an integrated Certificate in Community Health for Nurses (CCHN) from an institute recognized by the Indian Nursing Council or State Nursing Council, starting from the academic year 2020 onward. This applies to students who completed their final year of B.Sc. Nursing or second year of Post B.Sc. Nursing in 2019-20 or later. 2. Registration: Candidates must be registered with the UP Nurses & Midwives Council. If registered with another State Nursing Council, they must submit their UP registration certificate within 90 days of provisional joining, if selected. 3. Affidavit for Other State Registrations: Candidates registered with another State Nursing Council must provide an affidavit on a Rs. 100 non-judicial stamp paper at the District Health Society (DHS), promising to submit their UP Council registration within 90 days of joining. 4. Provisional Joining: Candidates joining as CHOs based on other state registrations will be considered provisional until they submit their UP registration certificate. Their official joining date as a CHO will start from the day of provisional joining. 5. Failure to Submit UP Registration: If candidates do not submit their UP Nurses & Midwives Council certificate within 90 days, their employment will be canceled, and they must pay a bond amount of Rs. 2.5 lakh to the DHS. 6. Current CHOs in UP: Candidates currently working as CHOs in Uttar Pradesh are not eligible to apply. | |||||

| 1. Photograph: Recent passport-size color photo with a light-colored, preferably white, background. Look straight at the camera with a relaxed expression. 2. Signature: Sign on white paper using a black ink pen. Only the applicant’s signature is accepted. Ensure the signature matches your actual one; otherwise, your application may be rejected. Scan only the signature area. 3. High School Mark Sheet. 4. Intermediate Mark Sheet. 5. All B.Sc. (Nursing) / Post Basic B.Sc. Nursing Mark Sheets. 6. Certificate from the Institution Principal verifying integration of the MLHP/CCHN course in Basic B.Sc. (Nursing) and Post Basic B.Sc. Nursing (as per Annexure VII). 7. University Certificate confirming the MLHP/CCHN course integration in Basic B.Sc. (Nursing) and Post Basic B.Sc. Nursing. 8. QR-Based COVID Experience Certificate from the district, issued in the format as per Annexure V. 9. Valid Category Certificate (refer to Point No. 4(d) if applicable). 10. Valid Photo ID and Address Proof. 11. UP Nurses & Midwives Council Registration Certificate or registration from another State Nursing Council. 12. Domicile Certificate. 13. Certificate for PwD, Dependent of Freedom Fighters, or Ex-Servicemen, if applicable. | |||||
